I understand that participation in classes at Motiv8 Dance Movement (MDM) includes physical activity such as movement, stretching, jumping, balancing, creative dance, conditioning, and tumbling, which carry inherent risks.
I accept responsibility for my child’s participation and release Motiv8 Dance Movement, its owners, staff, and affiliates from any claims related to injury, illness, or accidents occurring during any class, rehearsal, performance, or event.
I understand that MDM reserves the right to discontinue enrollment or Studio access for failure to follow Handbook Policies or for any reason deemed in the best interest of the Studio and that paid fees are non-refundable.
Payment & Chargeback Agreement
By submitting payment, I authorize Motiv8 Dance Movement (MDM) to charge the selected payment method and agree not to dispute or initiate a chargeback for any fees that comply with Studio policies. I understand all fees are non-refundable and that chargebacks made in violation of this agreement may result in immediate termination of enrollment and collection action for outstanding balances, fees, and administrative costs. In the case of any dispute regarding the terms, the prevailing party is entitled to fees and costs.
